Fraud Notice.

Several U.S. states require specific warning language to appear on insurance applications, claim forms, and other policy documents. The substantive warning is the same: knowingly providing materially false information to an insurer is a crime. The language varies by state.

This page collects the operative state-required notices. The specific notice applicable to a placement is also printed on the bound policy and on every claim form for the state of the seat.
